SURGICAL REQUIREMENTS

TO SCHEDULE AN APPOINTMENT, YOUR PET:

  • Should be in good health.
  • Should not be overweight.
  • Cats should be at least 3 months of age or younger than 5 years of age.
  • Smaller and medium breed dogs should be at least 6 months of age or younger than 5 years of age.
  • Larger breed dogs will need to be older than 1 year of age and younger than 5 years of age.
  • Dogs greater than 50lbs are only scheduled at the discretion of the veterinarian.
  • If a male, should have two testicles that can be felt upon examination.
  • If your female dog is in heat, we recommend postponing the surgery until 6-8 weeks after the last signs of heat. If you would still like to proceed with the surgery you will be required to sign a high-risk surgical waiver, and an additional cost will be charged.
  • We do not spay/neuter small animals such as rabbits.  We also do NOT spay/neuter certain *high risk* breeds such as Dobermans and brachycephalic breeds such as English or French bulldogs, Pugs and Persian cats.

Drop off is at 8:30 AM the morning of your appointment. Please allow up to 20 minutes for drop off as all surgery check-ins arrive at the same time. Dogs must be on a non-retractable leash and cats must be in a hard-sided carrier. Remember to bring proof of vaccination along if your pet is current on vaccines. Pick up is at 4:00 PM the same day in the same manner as drop off. Please allow time to go over post-surgical instructions with a member of our team.

Vaccination Terms

Pre-Anesthetic Blood Testing

Before undergoing anesthesia, a physical exam will be performed. It is recommended that blood work be done as part of this examination. Many conditions cannot be detected without blood work, and there is always a possibility that a physical exam alone will not identify underlying health issues. Having pre-anesthetic blood work performed can greatly reduce the risk of complications as well as identify medical conditions that could require medical treatment in the future. If you are interested in having pre-anesthetic blood work performed on your animal, we will have their blood drawn the week prior to their surgery. Due to the volume of clients, we see for our spay/neuter clinics, we are not able to accommodate blood draws on the day of surgery.

Animals In Heat

If your animals is currently in heat or her heat cycle has ended within the past 6 weeks, it is strongly recommended to delay her surgery until the uterus and ovaries have returned to their normal state. Being in estrus (in heat) can put your pet at an increased risk for intraoperative and postoperative bleeding. Being in heat will also increase your pet’s time under anesthesia and increase the size of her surgical incision. **Female dogs and cats that are either in heat or pregnant, are only spayed at the discretion of the veterinarian. This is non-negotiable.

Please note: All surgical patients will receive a small, green tattoo near the incision site. The tattoo will ensure that anyone examining your animal knows they have been sterilized.

Canine Clinic Services

Prior to Surgery

Pets must be kept indoors or confined the night before surgery. This ensures that they are not eating outside, which could potentially be dangerous during surgery.

Adult animals must have food taken away the night before surgery at 12 am midnight with no additional feedings prior to surgery. Animals four months or younger may have food. All pets are allowed to have water prior to surgery.

All cats must arrive in a cat carrier or humane trap if feral.

Dogs and cats that are at a higher risk of disease exposure (boarding facilities, day care, dog parks, socialization with other animals) should receive at minimum core vaccines. These are as follows:

  • Core vaccines for cats include rabies and feline panleukopenia(distemper)/rhinitis.
  • Core vaccines for dogs include rabies, canine distemper, parvovirus, and hepatitis.

Columbia Animal Shelter maintains the highest levels of cleanliness and disinfects all surfaces between patients; however, we cannot guarantee that your pet will not be exposed to contagious disease. Therefore, we strongly recommend that all pets visiting our shelter and clinic be current on their core and non-core vaccinations. It is important to understand that no vaccine is 100% effective.

We also recommend pets be protected from fleas with topical or oral preventative. If live fleas are seen on my pet they will be given a Capstar pill, at a cost of $10, to kill the live fleas as to not contaminate the clinic or other animals.

CPR Status Authorization

I understand Columbia Animal Shelter requires a CPR status prior to the start of all procedures so immediate action can take place in the event of cardiopulmonary arrest before, during, or after anesthesia, or anytime during our care. I acknowledge that the veterinarian or a staff member will make every effort to contact me regarding treatment in the case of this unforeseen event. The starting cost of CPR is approximately $200.

DNR means do not resuscitate. This is a decision that resuscitation (CPR) is not performed if the pet stops breathing, has no heartbeat, collapses, or becomes unconscious. Resuscitation (CPR) of a collapsed or unconscious patient is tailored to meet the needs of the individual. ONLY closed chest CPR will be performed. This may include any or all of the following:

  • Establishing an airway via insertion of an endotracheal tube and administration of oxygen or medications through the tube
  • Establishing intravenous access via insertion of an intravenous catheter and administration of fluids and injectable medications through the catheter
  • Chest compressions

Animals that have survived cardiopulmonary arrest and have been successfully resuscitated (CPR) are extremely critical and unstable. Management of the post-arrest patient requires vigilant monitoring and the technical expertise of dedicated critical care personnel. This care is costly, and the outcome is uncertain.

I have read and understood the information above or have had it explained to my satisfaction. I understand that the anesthetic, surgical or therapeutic procedures and treatments my pet is undergoing today may involve the risk of complications, injury or even death, from both known and unknown causes, and no warranty or guarantee has either been expressed or implied as to the result or cure. Hereby, I expressly agree to release Columbia Animal Shelter, and its agents and representatives, from liability for any and all damages to my pet and agree to hold Columbia Animal Shelter, its agents, and representatives harmless from any liability associated with the procedure and treatments being performed on my pet.

CPR Status Authorization

I accept that if the hospital staff is unable to reach me within 15 minutes after the initial CPR procedures, and after exercising reasonable medical judgment, determine that there is no hope for success, the staff will cease further CPR procedures. I understand that despite the best efforts of the veterinarians and staff at this facility, even the most successful CPR that may restore my pet’s life may not allow for my pet to regain his/her normal mental and physical health.

If I request such emergency procedures, I agree to be held financially responsible for veterinary services provided to my pet while staff members pursue treatment and try to reach me for further directions. Regardless of my pet’s recovery or survival, I agree to pay CPR fees in addition to other fees already identified by the practice and agreed upon by me.

Post Surgery & Recovery

The post-operative period is important for the healing process of your pet’s surgical procedure. You will be provided with recovery instructions at the time of discharge; however, please keep your pet calm and quiet for 10-14 days post operatively. No bathing, no running or jumping. Leash walks only for dogs. A cone collar is strongly recommended for all patients post operatively. The cost of this cone collar is an additional $15.00.

While you may make every effort to keep pets calm and quiet at home, there are some pets that are more excited and active than others. If this is the case, we can provide a mild sedative to go home for your pet. The cost of this medication is an additional $15.00. Please note that cats do not always require this additional medication, but it can still be beneficial.

Financial Responsibility

When scheduling a spay/neuter appointment, a non-refundable deposit of $50.00 is required to hold your appointment and will be counted toward your total bill once surgery is complete. Pennsylvania state law requires a current rabies vaccination for your pet. Please bring proof in the form of a certificate (tags and/or receipts are not acceptable forms of proof). If unable to provide proof of up-to-date Rabies, one will be automatically administered at a cost of $30.00.

I agree to pay in full for all services rendered at the time of discharge, including those deemed necessary for medical or surgical complications or otherwise unforeseen circumstances. I understand that a Rabies vaccine is required by PA state law once a pet reaches 12 weeks of age. If I cannot provide proof of a current Rabies vaccination, Columbia Animal Shelter has permission to give a Rabies vaccine to my pet at the cost of $30.00. I understand it is recommended that my pet also be current on all other vaccinations. Columbia Animal Shelter is not responsible for possible consequences if my pet is not current on their other vaccinations. I understand that it is recommended my pet be current on flea/tick medication. Financial Responsibility Terms

Surgical Consent

I am the owner or authorized agent of the pet named above and I am at least 18 years of age and have the authority to execute this contract. I hereby give Columbia Animal Shelter, its veterinarians, authorized agents, staff, and representatives consent and authority to perform surgery on the animal named above.

I certify that my animal is in good health and that if it is over four months old that it has not eaten since 12AM (midnight) last night.

I understand that it is recommended my pet be current on flea/tick medication. I also understand that if live fleas are seen on my pet, they will be given a Capstar pill, at a cost of $10, to kill the live fleas as to not contaminate the hospital or other pets.

I understand that a Rabies vaccine is required by PA state law once a pet reaches 12 weeks of age. If I cannot provide proof of a current Rabies vaccination, Columbia Animal Shelter has permission to give a Rabies vaccine to my pet at the cost of $30.00.

I understand it is recommended that my pet also be current on all other vaccinations. Columbia Animal Shelter is not responsible for possible consequences if my pet is not current on their other vaccinations.

I understand the benefits of spay/neuter and routine vaccination and understand that there are potential complications related to such procedures including vaccine reaction, persistent bleeding, infection, anesthetic side effects, surgical dehiscence, etc., and that Columbia Animal Shelter is not responsible for these complications.

I understand that Columbia Animal Shelter will perform a physical examination before surgery is performed. I understand that while I have the right to waive pre-operative bloodwork, doing so may prevent the detection of underlying illnesses and/or diseases which may compromise my pet's health during and/or after anesthesia.

I understand that some health conditions significantly increase surgical risk, including but not limited to, pregnancy, heat, and diseases related to the heart, liver, and kidneys.

I understand that if my animal is pregnant, in heat, has an undescended testicle (Cryptorchid), or any other abnormality, there will be an additional minimum charge to me of $50-$100. This is due to additional surgery and anesthesia time required to perform the procedure. I understand that if my animal is pregnant, the pregnancy will be terminated during surgery, and there will be a minimum charge to me of $40-$100.

I understand that if my animal has an open umbilical hernia, it will be repaired at time of surgery at an additional minimum charge to me of $100.

I have been informed that there are certain risks and complications associated with any operation or procedure, however rare, including injury and death. I further understand that during the anesthesia period and course of the operation, unforeseen conditions may arise that may necessitate the performance of additional procedures.

I authorize the use of appropriate anesthesia and pain relief medication as needed before or after the procedure, and I understand that there are risks associated with the use of any anesthesia and medication. I further understand that support personnel, including clinic staff, volunteers, and veterinary students, will be used as deemed appropriate by the veterinarian. I understand that qualified veterinary students, supervised by the veterinarian, may perform all or part of surgical procedures.

I understand that my animal will receive an approximately 1cm green tattoo on its abdomen as part of the spay/neuter surgery today. This may appear darker immediately after surgery but will fade to a faint line. This tattoo will provide an easy way to identify my animal as “sterilized.” I understand that all Trap Neuter Return (TNR) patients will receive an ear tip, in addition to the green tattoo.

I understand that Columbia Animal shelter strongly recommends the purchase of an e-collar and as part of every surgery. The e-collar must remain on as instructed to help prevent post-operative infection or complications, and that if I say I have one at home and do not purchase one at time of pick-up, and/or fail to ensure that my pet wears it properly, I will be financially responsible for any infections or complications that may arise as a result. This may result in over $1000 in veterinary fees at other hospitals/emergency hospitals. We do not provide emergency care.

I understand that if I do not retrieve my pet(s) at the agreed upon time I will be charged a boarding fee of no less than $25 per night. I further understand that if I do not claim my pet within two days of discharge time, Columbia Animal Shelter can assume that I have abandoned all ownership rights to the animal and has been authorized to place the animal up for adoption. I will remain responsible for any treatment and boarding costs. I understand that if I pick up my pet past the agreed upon pickup time, I will be subject to a $25 late fee.

I understand that Columbia Animal Shelter has the right to refuse service to any animal for whom surgery is deemed by a veterinarian to be at risk for surgical complications and/or other health risks, as well as too aggressive to handle by the staff.

I hereby release Columbia Animal Shelter, its veterinarians, assistants, volunteers, director, and employees from any and all claims arising out of or connected with the performance of this procedure or any adverse reactions my pet may have to vaccinations, medications, anesthesia, and/or the surgical procedure.

I will not hold Columbia Animal Shelter, its veterinarians, or any team member liable for any complications that may arise. I further agree to indemnify and hold Columbia Animal Shelter harmless for any damages caused during the transportation or treatment of the animal, and for any damages caused by any unforeseeable events including fire, vandalism, burglary, extreme weather, natural disasters or acts of God.

I have confirmed my pet's cardiopulmonary resuscitation (CPR) directive with a veterinary team member.

I agree to pay in full for services rendered today at the conclusion of my visit, and I understand that Columbia Animal Shelter does not offer any payment plans, credit, or client account. Columbia Animal Shelter accepts cash, checks and all major credit cards.
