Adoption Application Questionnaire

The average cost of care for a dog or cat averages between $500 and $1,000 a year, with higher costs for treatment of disease or injury. Are you financially ready and able to provide veterinary care for this animal?

Please note that in order to be considered for adoption you MUST allow CAS to contact your current veterinarian to conduct a reference check.

Cats and dogs require yearly physical exams. Do you agree to provide yearly physical exams and vaccines?
Do you agree to provide monthly flea, tick, and heartworm preventatives?

CAS requires personal background checks to be completed prior to being approved for adoption. Do you consent to allow CAS to complete a background check prior to adopting?
